医療本は経費で落ちるからそっちで買ってる。アマゾンポイントはゴルフ用品とか服みたいに経費で落ちそうにないのに使ってる。
漫画や雑誌も経費で買えばいいのに。
服だって診察の際に白衣の下に着ます。で大丈夫だよ。
漫画、雑誌はそうしてる。実際患者用に購入してるしな。服は厳しそうだからやめてる。
というか、経費で落とすならアマゾンなんかで買わないで実店舗で試着して買うかな。
アマゾン便利なんだが、服飾関係は弱い。

それじゃ先細りだ。週5ちゃんと開けるべき。
カネがないのなら日曜バイトや夜の泊まりバイトにしたほうがいい。
利便性にまさる他のクリニックに患者がうつってしまう。

皮膚科患者単価500点は指導行きだろうが、
レセ単価ならまだまだ余裕ありだな。
しかしこの表の653点は鵜呑みにできん。何故なら院内もまぜこぜでの
点数だから。院内なら600点で呼ばれるか呼ばれないかというところか？
馬鹿だろお前。俺がレスしたにレセ単価って書いてあるだろう。
ちゃんと人の話を聞きなさいと小学校で教わらなかったか？
そんなことだから勤務医時代から患者からのクレームがたえないんだぞ。
もっと取れるぞ。
それより皮膚科のみんな、
クレナフィン派？ルコナック派？ネイリン出してる？
クレナフィンとルコナックはほぼ同じだけだしてる。
新規の爪白癬で内服使うときはほぼネイリン。採血いらず。併用禁忌もない。これが楽でいい。
儲けを考えればラミシールで半年引っ張ったほうがいいだろうが、血管とりにくい爺さんに時間取られることを考えるとネイリンのほうがいい。
イトリゾールも併用禁忌があるから、内服中の薬剤聞き取りに時間取られるのが面倒。lion studyで効果がラミシールよりおとることもわかってるし。
出が悪いんだが出る時は一気にビチャーだろ。
すぐ無くなるとよく言われる。

内科の大変さを考えたら3割くらいでもいい位だ

どこまでが少ないスタッフ数なのかも人により違うし
一般内科よりはどう考えても少人数でできるだろう
そういうのの会話に付き合うだけで人手取られる
診療科による横の人手比較を言ってるんだが理解できないのかな

とアピールしたほうがよい。
まして事務員なんて、別にクリニックがどうなろうと知ったこっちゃないわけだから全く無意味。
自分でやる。
守れるのは自分しかいない。
一時間座って
「はい、はい、あははは。スイマセンでした。おっしゃる通りです。以後気を付けます。」
を繰り返していればOK。
経過観察で終了。
やましいことをしていない限りは弁護士なんて一切不要。
やましいことがあれば、その時は連れてけ。

待合室の漫画本は豊富で選択のセンスもよいです。
集団個別入ったことあるが、そのとおり。受付して会場で席に座って終わるのを寝て待っていればよい。
その後はレセ単価下げるのに腐心したからよばれることはなかった。だからよくわからんが、そのように聞いている。

待ち時間に漫画読みながら寝ていたらお茶を出してもらいました。漫画もいっぱいあります。
診療技術は低くひどいヤブですが時間つぶしには最高です。
受付の子をちょっと怒鳴りつけたらビビっていて面白かったです。
自己負担のない人には超オススメです。
ジャンプやマガジンは置いてないが。
待合室に日に焼けた表紙の汚い雑誌を置いてる医院はヤブ

余裕ではないだろ。
次年度は集団に呼ばれたその中から上半分が個別に呼ばれるんだから。
集団に呼ばれた人はみんな次年度の点数を下げようと必死になる中で自分も点数を減らさないといけないので、上位半分以下になるのはタイヘンと思いますよ。
個別指導は性悪説で突っ込んできます。
集団指導自体はなんともなくても、呼ばれたこと自体が脅しです。
内覧会を何度すれば気がすむんだ。

当地では、内科はほぼ半分が個別行き

右派ってきたら指導対策で電カルから紙カルテに変える人もいるみたいだね。
右派ってバイト雇うようになると、紙カルテが圧倒的に有利
最強コンボ。
眼科は紙カルテが分かりやすい。
あれか。処方や検査は電カルで診療記録だけ紙カルテってやつ？
電カルと併用期間を設ければ可能じゃないの?モニター見ながら紙カルテに続きを書いていく。
５年たったら電カルを終了させれば法的にも問題ないんじゃないかな。
電カル、レセコンなど役所が勧めてくるのは向こうが管理しやすいだけ
マイナンバーでもそうで結局国民の利便性など考慮してない
まぁでも数が増えると大変だからなぁ
定型的な請求してるだけのところはオンラインがいいかも
期間縛りで請求要件に厳格な周期性があったりする場合は、紙が神
常にスキャンしておけば、普通の紙カルテとして運用して、
古い部分が邪魔だと感じた段階でいつでも捨てることができる。
で終わりですが。
電カル画面に視線移してマウスでdoボタン押すより速いわ。
数十秒の違いでも1日数時間違うことになる場合もあるからね。右派なら紙がいいよ。
職員の負担なんてちょっと時給あげれば納得するし、
カルテ置く場所なんて些細なこと。昔のカルテは二度と見返さないんだから倉庫でホコリ被ってようが知ったことではないよ。引退するまで放置でいいでしょう
オンラインはどこまでも遡られて分析される
気味が悪いよ
紙カルテの頃これやってたな。しかし、薬剤買えたときに①にダッシュつけたり、さらにかえたらダッシュを2つつけたりして、汚くなってきたからと「今回より新しくナンバリング」と書いて書き直したりする。
そしてあとで①が一体何だったのかわからなくなったりしてなんだかぐちゃぐちゃになっていくんだよね。
患者が「いつものをください」と言ってる間はいいけど、「何々はあまってるからいりません。」といわれると、その薬のナンバリングがわからないからカルテを後ろから繰っていく手間も面倒だったな。
倉庫もそんなに余裕ないよ。俺は43坪のテナント借りてるけど余分なものを積んでおくスペースなんてあまりないよ。
田舎の戸建てだと、電子カルテなんか意味ない。紙で十分。
俺４０歳だけどそう思うわ。
工夫が足らん。
そんなことしてたら回らんわ笑
オンラインソフトが主流の流れで、
ハードやセッティングは自前でしてくださいが増えてきてる。
導入時にケチって中途半端なスペックのPC導入すると痛い目見るよ。
高スペックPC購入しとけ
セッティングからその後のトラブルのフォローまでまるまる見てくれるところでなきゃ面倒だし、解決できないときが怖い。
趣味のパソコンいじりじゃないんだからな。
電子カルテいれるなら少々高くてもフルサービスのほうがいいな。実際俺はそうしてる。
オンラインカルテだとしてもフルサービスを用意してる業者はいっぱいありそうだけどな。
とにかくカルテなんかに金使いたくない人はサービスの薄い業者の電子カルテ使うぐらいなら紙カルテがいいと思う。
そのほとんど全てがお役所誘導の既得権システムの構築だから
本来ならせっかくもっと便利になる筈の仕組みなのにかえって窮屈になったり融通性が低下する
マイナンバーなんてその典型
一元管理で本当にその機能をフルに活かせるなら公務員も削減できるに違いないし
カスみたいな関連会社も必要なくなるのが自然なんだけど、そうはならないし減税にもならない
イオン某、節税電話に注意しましょう。
そもそもの処方量が多すぎだったんだと思うよ。俺は全くきられていない。
ヒルドイド内科で処方してるが切られたことない。
因みに1回(1ヶ月)にヒルドイド何グラム処方したらどの程度切られた？

ここには脱毛もあって一年通ってました。
脱毛あと、「背中も毛深いね」と脱毛の延長を誘発するような言い方をされました。

開業して良かった事の一つは合わない患者は他所に行くので、総合病院の時みたいにストレス溜めながらみなくて済む事だな。
いや、定期的に新規の基地外患者は来るし、不満そうなのに何年も通院する変な奴もいる。
変なのは、病態にもよるが、基本1回目みたあと、再診の指示はしない。

勤務医時代に並ぶのは返済終る7年後だ。
それまでは勤務医時代の手取り 2/3
そこから開業ローン払うとほぼ手取り半分
会議や研修医指導は無くなったが、それを上回る事務仕事。
3年目でやりがいすら感じなくなった。
継承業者と面接したが、ほぼ固定資産代としか評価されなかった。継承して精算してもまだフェラーリ代位借金残る。どうしたものか。
なぜ持ち物件でやらなかったのか
都内ビル診より郊外持ち物件、採算ライン１０００万は下げられる
まず開業前に5000万貯めて、受付カウンターと看板だけで無借金スタートですよ
うちもそんな感じ。院内だけど。
2年目。場所が良かった。
黒字化してしまうのでどうしよう。
卒後8年目です。
開業準備期の勤務医の頃は気付きにくいけど
いくら銀行金利が激安でも長期の融資だと、融資元本に対する支払う利子はかなりの割合になる
計算してみたら１億の融資で支払う利子総額が２千万近くいってたわ
無借金持ち物件なら、勤務医の半分以下の労働で同等以上の手残りになる
場所以外の設備は、開業後いつでも追加できる
そして、開業してしまえばそんなものは不要だと気付く
設備よりも人員よりも、場所決めの方が遥かに大事で、オマケに自分の時間と身体リソースが有限だと気付くからだ
余計に稼ぐより、節税に労力割く方がQOL上良いことに気付く
ほんとにそうですね。
設備は中古で充分。
